Health forms
Health forms must be on file for all children and must be updated with new information from your pediatrician every year. Your child will not be permitted to attend Roots and Wings until an up-to-date health form has been submitted, though you will remain liable for tuition payments as scheduled. Returning families must also submit a new health form at the beginning of the year. In addition to immunization records and general medical information, you must include your current health insurance policy number(s) on your child’s medical and emergency information forms.
Illness
Your child should not attend Roots and Wings if he or she has:
- A temperature over 100.4 within the past 24 hours
- Vomiting
- Diarrhea
- Conjunctivitis
- Congestion that interferes with breathing or sleeping
- Early signs of a contagious disease (chicken pox, impetigo, etc.)
- Head lice/nits
In general, do not send your child to Roots and Wings if he or she is unable to participate in all aspects of the program, including outdoor activities. Parents and staff must consider the well-being of all of the children. If the staff feels that a child should not be in attendance, the child’s parents will be called and asked to remove the child as soon as possible.
It is important to keep in mind that children who spend time in the company of other children are exposed to more minor illnesses than those cared for alone. If your child has been in an extra virus or two, especially in the winter.
Parents are required to keep children at home during the contagious period of more serious illnesses; however, this may not be possible with diseases such as chicken pox, in which symptoms appear after the contagious period has begun. In general, you should notify the teachers if you know that your child has been exposed to a contagious illness. Further, you should plan to remove him or her from Roots and Wings during the anticipated contagious period of any severe illness for which there is no immunization.
